Step 1
Please indicate which membership is appropriate for you.
A Clinical Investigator Membership lists investigators by their specialty and therapeutic areas of expertise. The annual fee for a Clinical Investigator
Membership is $325. A Facility Membership emphasizes the expertise of the investigative site. The annual fee for a Facility Membership is $425.

 

 

 

 

 

 

Step 2
Select Individual Investigator Personal Data Form or Data Form Facility/Site Member. Please download and print the membership forms. You may either send the forms via email info@clinicalinvestigators.com or Fax to 763.544.1415.

 

 

 

 

Step 3
After receiving your membership forms, we will send via US mail a copy of your listing proof for your approval.
